THERAPIST – CREATE PROGRAM

JOB SUMMARY

Provides in-home, office and telehealth outpatient mental health assessments, counseling services, and provider education and consultation in the CREATE program (Co-designing Rich Environments for All to Thrive Everyday) : The target population includes children and their parents/guardians/caregivers (families) who are referred for emotional, behavioral and developmental concerns and may have complex medical needs and youth transitioning from pediatric to adult care (HCT), and LGBTQ+ children and youth. They must reside in Duval County. Therapists will provide services to stabilize their family, parent-child relationships, mental health or prevent entry into formal systems of care.

Minimum Qualifications:

Education

Master’s degree in Social Work or Mental Health from an accredited college or university. Eligible to become a registered intern with the State of Florida

Experience

One year of counseling experience is preferred.

Licensure, Certification, and/or Registration

Licensed Applicants are preferred. Meet training requirements stipulated by the State of Florida to become a registered intern who is working toward licensure.

Primary Job Functions

Provide individual counseling services as needed to clients referred to this program. Services may be a combination of office based, in-home or virtual as needed.

Arrange for weekly clinical supervision if working as a registered intern.

Document all counseling sessions in the JFCS automated system.

Participate in meetings with the program Director as needed.

Provide counseling as needed and consult with Licensed staff on the need for intensive services.

Complete reports required by JFCS and Project CREATE to determine the number of counseling sessions provided for this program.

Meet with Director to identify training needs

Participate in staff meetings as required.

Consult with the Director of Project CREATE to ensure that appropriate services are being provided to the children and families.
