Service Areas: We service 25 communities in southern NH from Hooksett to Nashua. We are the preferred provider for 3 hospitals. We pay travel time in between locations and reimburse .625 cents a mile!

Responsibilities:
Ensure the comprehensive coordination, continuity of health care and prevention of further health complications as patients transfer between care settings to service with HHHC. Responds to hospital/facility patient referrals in a timely manner building positive relationships with referral partners, demonstrating HHHC's value through positive patient outcomes and seamless collaboration. The position requires in person meetings with patients and families in a hospital as needed. Additionally, they will participate in community collaborations with hospital partners.
Communicate with referral case managers and others to obtain clear assessment of patients needs.
Gain strong understanding of patient's clinical status, home support and goals of care.
Request comprehensive clinical data to be available to HHHC Intake and TCN for review.
Educate patients and families as well as referral partners on Home Care and Hospice and what to expect with services.
Communicate with Home Care/Hospice Managers re. safety concerns for clinical staff prior to accepting referral.
Complete TCN Note using Home Care/Hospice Template.
Advocate for patients/families to ensure that needs and goals are represented.
Track pending list to ensure that all required documentation was received in order to hand off referral to appropriate clinical department for scheduling of admission. Qualifications
Associates or Bachelors of Nursing Degree.
Registered as RN or LPN with New Hampshire Board of Nursing or Compact license.
Two years minimum community / public health nursing.
Marketing / sales experience preferred.
Microsoft office suite skills are required. Required
Satisfactory criminal record.
Satisfactory motor vehicle record.
Bureau of Elderly and Adult (BEAS) clearance.
