Beacon Medical Group (BMG) is seeking a compassionate and skilled Nurse Practitioner to join our hospital-based Physiatry team.
This role focuses on delivering high-quality care to patients transitioning to rehabilitation, skilled nursing facilities, or home settings.
The position includes inpatient coverage in the rehabilitation unit and collaboration with multidisciplinary teams to provide comprehensive, patient-centered care.
Key Responsibilities Clinical Care: • Provide primary and nursing care in collaboration with the Practice Site Physician(s) or designee and under the direction of the Practice Manager/Director.
• Conduct patient rounds, consultations, and transitional care planning for those moving to rehabilitation or other care settings.
• Perform thorough medical histories, physical examinations, and interpret diagnostic results to develop comprehensive care plans.
• Order and interpret laboratory and diagnostic tests; administer medications and therapeutic treatments as needed.
• Educate patients and families on health maintenance, rehabilitation strategies, and preventive care.
Administrative Support • Maintain accurate, complete, and timely documentation in patient records and charts.
• Participate in interdisciplinary care conferences, assist with referrals, and ensure seamless continuity of care.
• Contribute to developing, implementing, and monitoring clinical protocols, policies, and procedures.
• Support performance improvement initiatives to enhance quality metrics and patient outcomes.
Education & Training • Mentor and collaborate with nursing students, medical residents, and pre-professional trainees.
• Engage in ongoing educational programs to maintain clinical expertise and provide evidence-based care.

Qualifications Education • Bachelor’s degree in Nursing required; Master of Science in Nursing preferred.
Licensure • Current RN license in Indiana and national certification as a Nurse Practitioner.
Experience • Minimum of 2 years of clinical nursing experience; new graduates must complete a six-month Nurse Practitioner internship or equivalent training.
Certifications • Current BCLS (CPR) certification is required.
• Prescriptive authority or the ability to obtain within six months of hire.
Skills • Strong clinical and analytical abilities to assess and manage patient care effectively.
• Excellent interpersonal and communication skills, both verbal and written.
• Proficiency in care planning, patient education, and interdisciplinary collaboration.
• Bilingual skills (speaking/writing) are a plus.
Working Conditions • Primarily hospital-based, with time spent in inpatient rehabilitation units.
• Requires physical stamina for prolonged periods of walking/standing, lifting/positioning patients (up to 35 pounds unassisted), and performing CPR as needed.
• Exposure to biohazards and typical clinical environment demands.
