Job Description
Develop, organize and implement a consistent transition services for students. Encourages and leads staff involvement in instructional planning and decision making leading towards successful transition to adult life for students in schools.
Provides direction and supervision to the Transition Resource Specialist.
Assists Principal in contacting/setting up work settings for career exploration. Assists in organizing building jobs/creating student work schedule. Assists in contacting/setting up volunteer opportunities for students.
Conduct discovery activities with students to help identify preferences, interests, needs and services after completion of school. Research and obtain information regarding what options are available for students, incorporating this information into the transition process.
Works one-on-one or in small groups with students to practice/teach job related skills at school and in the community. Schedules Smart apartment and keeps the space equipped with necessary items for student learning.
Provide parent education regarding transition and guardianship. Provide support to students in learning personal hygiene and social skills acquisition. Provide assistance with student and family tours of day programs available for our graduating students. This may include attending trial days with students. Assist teachers in developing transition goals for annual IEPs. Attends IEP meetings and assists school staff and parents/providers with future employment-related goals for students.
