Position Overview: We are seeking a dedicated and experienced Hospice RN Case Manager to join our team. In this role, you will coordinate and provide skilled nursing care to hospice patients in their homes, facilities, or other care settings. You will be an integral part of an interdisciplinary team, ensuring the highest standards of care and compassion are delivered to our patients and their families.
Key Responsibilities:
Conduct comprehensive nursing assessments to develop and implement individualized care plans.
Coordinate care with interdisciplinary team members, including physicians, social workers, and chaplains.
Provide direct patient care, including symptom management, medication administration, and pain control.
Educate patients and families about the hospice process, disease progression, and care techniques.
Facilitate communication between patients, families, and the care team.
Maintain accurate and timely documentation of patient care and communication in compliance with regulatory standards.
Participate in on-call rotations on holidays to ensure 24/7 patient support.
Advocate for the needs and wishes of patients and their families.
Qualifications:
Current and unrestricted RN license in the state of Arizona.
Minimum of 1-2 years of nursing experience; hospice or palliative care experience preferred.
Strong clinical skills with a focus on pain and symptom management.
Excellent communication and interpersonal skills.
Ability to work independently and manage a caseload effectively.
Reliable transportation, a valid driver’s license, and current auto insurance.
