Coordinate care for Disabled and Older Alaskans under Division of Senior and Disability Services Medicaid Waivers. Responsible for screenings, developing and amending Plans of Care, and accurate billing, ensuring ethical practices and team collaboration.
Preferred Associate degree in Human Services or related field; high school diploma required. Minimum of 3 years human services experience, with cross-cultural understanding. Willing to travel throughout western Alaska. Bilingual in Yup’ik/English preferred.
Must hold a DSDS care coordination number. Provide licenses and certifications; complete TB clearance, CPR, and First Aid as required.
Experience with case management, native programs, and Division of Senior and Disability Services regulations preferred. Skilled in scheduling, documentation, and computer applications.
