POSITION SUMMARY: (WHAT, HOW, WHY)
The Case Manager delivers transformative case management and job coaching services to program participants. This involves monitoring cases by verifying attendance, evaluating service plans, and advocating for supportive services to prepare participants for work. They assist with intake, retention, and billing processes. MINIMUM REQUIREMENTS:
Education and Experience: Bachelor's Degree in Social Work, Human Services, Vocational Rehabilitation or related field.
Knowledge: Knowledge of and sensitivity to the unique needs of the target population - justice-involved, unemployed, and underemployed clients. Must be an effective communicator, critical thinker, and problem solver. Documented skill in areas of analyzing information, decision making, research skills, verbal communication, written communication, resolving conflict, workflow implementation, human relations, time management, and integrity. Ability to motivate others toward achieving goals.
ESSENTIAL DUTIES:
Conduct Intake and assessments to identify participant needs and strengths.
Develop and implement individualized career and financial plans.
Coordinate and facilitate program services, such as job readiness training, financial literacy workshops, and housing assistance.
Collaborate with external partners to provide additional support and resources.
Monitor participant progress and provide timely interventions. PERIODICAL DUTIES:
Maintain accurate and up-to-date participant records in the agency's data management system.
Compile and submit required reports, including progress reports, success stories, and program evaluations.
Provide post-placement support, such as job coaching and career counseling. OCCASIONAL DUTIES:
Assist participants in developing job search strategies, including resume writing and interview preparation.
Connect participants with potential employers and job opportunitiesWORKING CONDITIONS: (SITTING, STANDING, WALKING, OVERTIME, ETC.)
Must be able to sit for extended periods of time to complete reporting and computer work.
