Description
Greater Nashua Mental Health is looking for an Intake Support Specialist to join our team! The Intake Support Specialist provides short-term, office-based and community-based support to adults who are seeking recovery services from a mental health condition and other co-occurring disorders. May occasionally transport clients or meet in the community. Provides community support, case management, Illness Management Recovery, Intentional Peer Support, and/or other services to assist individuals in developing strategies and skills to cope with aspects of their mental illness during the period between intake and treatment team assignment. Provides intake director with updates about important changes to clients' status as needed to effectively triage waiting list. Assists intake staff with closing procedures for clients who no longer need services or who fall out of touch with GNMH before treatment team assignment.
Duties & Responsibilities:
Engages clients in case management and/or targeted interventions, including evidenced-based interventions (i.e., IMR, Intentional Peer Support, Motivational Interviewing), aimed at supporting improved functioning in the community
Conducts regular follow-up calls to clients during period before treatment team assignment
Provides intake director with updates about important changes to clients' status as needed to effectively triage waiting list
Assists intake staff with closing procedures for clients who no longer need services or who fall out of contact with GNMH before treatment team assignment
Completes all documentation for each service delivered, outreach attempts, and team discussions within 3 business days (with first day being date of service/event). Strives to complete documentation collaboratively with clients so notes are completed on the same day of service
Attends scheduled individual supervisions, group supervisions, and required trainings as part of professional development and to enhance client care
Completes and submits mileage/expense sheets within the month they are due
Performs other duties or special projects as required or as assigned Requirements
Qualifications:
Associate's Degree plus 2 years work experience in related field; alternately Certified Peer Support Specialist
Bachelor's Degree in Psychology or other related field, is preferred
Basic education on mental health conditions
Understanding of the components of recovery
Strong collaborating skills and an ability to work effectively both independently and as part of a team
Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive
Excellent communication and interpersonal skills
Ability to multitask, and be flexible with regard to workload and assignments
Ability to exercise judgment
Knowledge of Microsoft Office products (Email, Word, EXCEL)
Must possess a valid driver's license and automobile with liability insurance in the amount required by agency policy
