Advanced Practice Provider (Nurse Practitioner or Physician Assistant) - Northern Express Care Newport

Northern Counties Health Care is seeking a per diem Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Northern Express Care team in Newport, Vermont. As a key member of our urgent care team, you will provide high-quality, patient-centered care in a fast-paced environment.

Responsibilities:

Provide primary and urgent care services to patients of all ages within scope of practice.

Conduct health histories, physical exams, diagnosis, treatment, and patient education.

Order and interpret diagnostic tests according to protocols and clinical judgment.

Prescribe appropriate medications and treatments following state and federal guidelines.

Maintain thorough and timely documentation in the Electronic Medical Record (EMR).

Ensure continuity of care through follow-up and coordination with primary care providers and specialists.

Participate in quality improvement activities and clinical meetings as needed.

Maintain patient confidentiality and adhere to infection control policies and safety standards. Qualifications:

Graduate of an accredited Nurse Practitioner or Physician Assistant program.

Current Vermont license and national certification required.

DEA license appropriate for scope of practice.

Experience in a primary care or urgent care setting preferred.

Strong clinical assessment and critical thinking skills.

Proficiency with EMR systems and basic office technology.

Ability to thrive both independently and as part of a team in a busy care environment.
