Position Summary

Vinson Hall Retirement Community (VHRC), a premier Continuing Care Retirement Community located in beautiful McLean, VA, is currently seeking an energetic and compassionate Registered Nurse (RN) to join our team in a PRN / On-Call capacity. As an RN, you will work closely with our dynamic Vinson Hall at Home Health team to deliver high-quality skilled home health nursing services to seniors residing within our community.

Requirements

Current RN license in the state of Virginia (required)

CPR certification (required)

Minimum of 1 year of experience in home health or a healthcare setting

Strong knowledge of OASIS documentation and familiarity with Start of Care (SOC), Resumption of Care (ROC), and Discharge documentation

Excellent communication and interpersonal skills, with the ability to collaborate effectively with physicians, patients, and team members

Ability to prioritize and manage time effectively in a dynamic environment

Compassionate, patient-centered approach to care Primary Responsibilities:

Complete Start of Care (SOC), Resumption of Care (ROC), and Discharge OASIS documentation in accordance with established guidelines and regulations.

Conduct detailed assessments of patients, leading to the development and implementation of personalized Plans of Care.

Provide direct nursing care to patients in their homes, following physician orders and adhering to all healthcare guidelines.

Collaborate with team members to ensure that the Plan of Care is consistently followed and adjusted as needed for optimal patient outcomes.

Communicate effectively with referring physicians, delivering timely and accurate treatment notes, patient progress, and any relevant changes in care.

Prepare for patient discharge by coordinating plans with the patient, care team, and physician as soon as possible to ensure a smooth transition.

Perform other duties as assigned, ensuring a high standard of care and customer service.
