Job Description

A Day in the Life of an Ambulatory Clinical Pharmacist: The Ambulatory Clinical Pharmacist supports Cheyenne Regional Medical Group practitioners. The ambulatory clinical pharmacist will provide clinical pharmacy services to improve patient care, patient satisfaction and provide efficiencies within the specified clinics.

Here Is What You Will Be Doing:

• Review medication refill requests for accuracy, timeliness and appropriateness against current charting and information.

• Review and update patient medication profiles and maintain patient medication profiles in electronic medical record.

• Review patient pharmaceutical treatment regimens to meet treatment guidelines and standards taking in account compliance, efficacy, safety and cost and provide recommendations to practitioners in a timely and appropriate manner and provide patient and caregiver education.

• Review medications for practitioners and patients to assess third party payor implications, necessary prior authorizations, ability for patient to obtain and utilize, assess for side effects or other barriers of pharmacy related compliance issues. Provide patient specific pharmacotherapy recommendations to the practitioner to improve patient compliance and medication availability.

• Educate the patient on the purpose of medication, proper use of medications, possible side effects and other required and important information.

• Provide staff and practitioner education regarding medications, therapy guidelines and other developments in pharmaceutical care and disease management with a goal to improve patient care outcomes.

• Create and provide medication synchronization and adherence service to patients.

• Provide medication and disease state management services including a comprehensive education program. Responsible for proactively reviewing patient metrics and goals and providing expertise to meet those goals.

• Create and maintain a collaborative practice agreement as outlined by the Wyoming state Board of Pharmacy. Review and update collaborative practice agreement annually or when requested by leadership.

• Coordinate antimicrobial stewardship activities resulting in optimizing antimicrobial therapies.

• Responsible for the oversight and pharmacy management of medication samples when applicable. This includes, but is not limited to, managing medication outdates and adherence to regulations, policies, procedures and standards.

• Document clinical interventions appropriately in EHR. Track, analyze and report program metrics and progress annually or as requested by leadership.

• Maintains professional and personal competency and complies with educational requirements

Here Is What You Will Need:

• PharmD or Bachelor of Science in Pharmacy from an ACPE accredited School of Pharmacy

• Registration and licensure current with the Wyoming Board of Pharmacy prior to start date

• Cheyenne Regional AHA RQI within 14 calendar days of start date

Nice to Have:

• Experience as an Ambulatory care pharmacist

• Experience working within a pharmacy collaborative practice agreement

• Completion of a ASHP certified Ambulatory care program or Board Certified Pharmacotherapy specialist
