Position Summary

The Quality Coordinator plays an active role in managing and facilitating WellSpace Nexus’s Quality Plan. Responsibilities include assisting with the development, implementation and evaluation of projects to improve the quality metrics; facilitating problem solving and developing positive working relationships at all levels. Will manage information related to UDS and HEDIS measures. Will work directly with the Quality, Medical, and Operational staff, and Health Plans to identify patients that have gaps in care and to engage patients in seeking appropriate care. The Quality Coordinator will play an active role in interventions that close the care gaps and improve outcomes for patients. Will support population health efforts and strategies.

Required Qualifications

Undergraduate degree in Public Health, or a health-related field preferred.

At least 1 year of experience in clinical quality improvement, healthcare quality management, or a related field. Equivalent experience in a clinical setting where you contributed to improving care processes, monitoring quality metrics, or supporting quality assurance initiatives will also be considered.

Medical setting knowledge preferred with experience in primary care settings or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.

Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, and/or physiologically impaired.

Compliance with all state and federal laws and regulations as they pertain to position including infection control, sexual harassment, mandated reporting, scope of practice, OSHA, etc.

Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.

Assist and support the Center/Department/Program to meet standards of High Reliability.

Strong customer service experience including active listening, problem solving, and the ability to remain calm in emotional or stressful situations.

Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.

Excellent interpersonal skills.

Ability to work extended hours and/or weekends related to Health Center hours of operation or special events and programs.

Fluency in additional languages preferred.

Essential Responsibilities

Under the direction of the Utilization Management Director and Chief Medical Officer, assists with planning, designing, implementing, and maintaining a comprehensive quality improvement/quality assurance program including targeted clinical interventions.

Assists as needed in huddles and chart scrubbing work flow processes to identify and outreach to patients with gaps in care or identify missed opportunities

Meets with internal and external audiences to identify and problem solve QM issues.

Supports the QM Committee’s quarterly meetings.

Coordinates the effort to gather data and prepare reports to meet the requirements of the WellSpace Nexus’s quality plan.

Evaluates variance and other data to identify QM opportunities to improve clinical outcomes.

Monitors WSH, HEDIS and UDS quality measures to identify corrective actions as needed. Reports on results.

Assists with related medical record data collection.

Assists with clinical quality projects and initiatives.

Attends trainings and staff meetings as required and performs related work as assigned.

Maintains and adheres to confidentiality, and privileged communications (patient, employee, and organization).

Assists with health plan audits.

Runs monthly, quarterly reports from Power BI.

Performs other duties as assigned.

Physical Demands and Work Environment

The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.

The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
