Job Description
Responsible for conducting risk/needs assessments and developing service plans for all clients assigned to their caseload, develop resources for clients and, facilitate workshops, programs and groups for clients.
Scope of Duties:
Assessment of all new clients via the approved agency Assessment Tools to determine counseling needs and appropriate level of care.
Develop Service Plans on all assigned clients based on approved agency assessment tools that is client centered and recovery oriented with client input.
Obtain previous records that are relevant to current treatment recommendation.
Monitor and document client progress according to funding source standards and/or operational policy and procedures.
Review, reassess and revise service plan with client input to address on-going problems and to determine need for continued services or discharge.
Schedule and facilitate individual sessions with clients as required.
Identify and/or develop resources and link clients to needed services.
Provide active case consultation and coordination when necessary with client linked resources and referral sources.
Facilitate scheduled workshops/programs/groups addressing client's needs.
Develop appropriate discharge plan on all assigned clients as per contractual requirements.
Facilitate house meetings as needed.
Participate in required internal/external Treatment Team meetings.
Complete all required reports and other documentation.
Demonstrate competency in program related certifications.
Provide support to Treatment Assistant function.
Interact with clients, staff, and outside contacts with respect and courtesy.
Maintain client and staff confidentiality in compliance with administrative policies and procedures.
Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions.
Other duties as assigned.
