We are looking for…
We seek a dedicated and compassionate Bereavement Case Manager to join our team. This role provides confidential intake support and direction for incoming family phone calls remotely and in person. Responsibilities include data entry into the call log, completing intake/enrollment and assessments, and documenting all information in Client Track or other software. The case Manager will support the planning and execution of monthly support groups, including developing and printing invitations and programming materials. They will facilitate appropriate referrals for grief support and bereavement information, maintain the delivery board of urns and caskets, and ensure care packages and caskets are delivered to the family's chosen funeral home. The role involves ongoing 30/60/90-day follow-ups with families via telephone or email, distributing grief support materials, and closing enrollments each quarter. Additional duties include distributing follow-up cards to families referred by community partners, supporting the Annual Memorial/Butterfly Release Ceremony, maintaining an inventory list, purchasing sympathy gifts and urns, and filing funeral contracts. The Specialist will attend community outreach events, assist with agency-sponsored events, and contribute to fundraising efforts. This position requires a commitment to supporting families through their bereavement journey and performing various tasks and special projects as assigned.
Required Experience and Skills
Ability to provide emotional support and understanding to grieving families.
Skilled in listening attentively and empathetically to clients' needs and concerns.
Strong verbal and written communication skills to effectively interact with families, colleagues, and community partners.
Ability to manage and support families in crisis situations with sensitivity and care.
Proficiency in managing multiple tasks, keeping accurate records, and maintaining organized documentation.
Awareness and respect for diverse cultural backgrounds and practices related to grief and bereavement.
Ability to assess situations, identify issues, and develop effective solutions for families' needs.
Efficiently prioritizing tasks and managing time to meet the needs of multiple families.
Working well with other team members, community partners, and external organizations to provide comprehensive support.
Emotional strength and self-care practices to handle the emotional demands of working with grieving families.
Ability to work effectively in a team environment and contribute to a positive organizational culture.
Bachelor's degree in a relevant field
2 years of related experience working in case management or community outreach services within community programs serving families dealing with grief and loss
Bilingual English Spanish Required
Valid Texas Driver License
Job Duties
Provides administrative and data entry support to the Director of Bereavement for the Bereavement Program. Assists in daily program needs. Performs specialized case management services per agency policy and procedures. Provides case management services, manage caseloads, complete client paperwork, and attend community events. Assists families struggling with the decisions and costs of where and how to prepare for a funeral, including funeral home referrals and financial assistance. Oversees ongoing follow-up to families who have suffered the loss of their child/grandchild to any cause via telephone contact, the distribution of grief support materials, and sending referrals to the counselor of their choice.
