About Us
Pathway to Wellness is a small, dedicated private primary care practice located in downtown Baltimore, MD. While we serve a variety of private patients, our focus is primarily on providing medical support to community-based partners, including assisted living facilities and rehabilitation programs. We work collaboratively with partner organizations to ensure high-quality, coordinated care is provided to their residents and clients.
Position Summary
We are seeking a proactive, organized, and compassionate Licensed Practical Nurse (LPN) to join our team on a PRN basis. This is a hybrid position that combines in-person community visits with remote work, including documentation and virtual meetings.
During community visits, the LPN will travel to partnered residential facilities (such as group recovery homes or assisted living facilities, all located in downtown Baltimore) to assess patients, take vitals, and gather medical concerns. After in-person visits, you’ll return home to complete documentation and communicate findings to the supervising Nurse Practitioner and administrative staff.
The role may also include occasional visits to assisted living facilities or accompanying the Nurse Practitioner on community assignments, but these are on an as-needed basis.
Key Responsibilities
Conduct in-person visits to partner residential sites (primarily two group homes in downtown Baltimore):
Take vital signs and document patient assessments
Identify any patient concerns or clinical changes
Communicate important clinical information to the Nurse Practitioner
Notify administrative team if follow-up actions (e.g., referrals, medical record requests) are needed
Monitor and track medications, especially in partnership recovery environments
Draw labs and substitute for the phlebotomist when needed
Complete all documentation and charting remotely following in-person visits
Attend weekly virtual team meetings (approx. 2.5 to 3 hours) and in-person meetings as needed
Perform pre-appointment patient check-ins to confirm visits and gather relevant clinical updates for the provider
Visit our Baltimore office approximately twice a month for team collaboration or training
Retrieve and review documentation for new admissions and recent discharges to support continuity of care
Completing audits
Accompany Nurse Practitioner to occasional assisted living site visits (as needed)
Work Environment & Schedule
• Hybrid model: In-person visits to patient sites + remote documentation + virtual meetings
• Visits typically occur once or twice per week, with documentation done from home
• No full-day travel: Visits include two rehabilitation center a Men’s home and a Women’s home
• Weekly team meetings are virtual unless otherwise noted
• Office visits required approximately 2x/month
• First week is fully remote, including training, orientation, and staff introductions
Qualifications
Active Maryland LPN license
CPR certification (current)
Valid driver's license and access to reliable transportation
Ability to draw labs and follow standard collection procedures
Previous experience with community-based care, substance abuse facility, behavioral health, or assisted living settings preferred
Comfortable using digital tools for documentation and virtual collaboration
Strong communication and critical thinking skills
Self-motivated and capable of working independently
