Position Overview:
Under the guidance of the Chief Clinical Officer and Indiana market President, the RN Case Manager collaborates with local and market teams to assess, develop and revise care plans for our clients.
The RN Case Manager will respond to client needs as identified by caregivers and others and identify community, payer and provider resources to meet those needs. The RN Case Manager collaborates with internal and external stakeholders and workgroups to identify areas of need and opportunity across the organization.
Responsibilities:
Reviews client needs/referrals as identified by caregivers and others conduct additional assessments and develops a care plan to address those needs. Ensures appropriate follow-up occurs.
Performs initial and ongoing assessments of selected, high-risk clients based on Agency policy and standards of practice to ensure effective and appropriate home care services.
Effectively partner with other stakeholders in the development of models to reduce the risk of falls, institutionalization, polypharmacy, social isolation/loneliness and similar adverse events.
Participates as a key stakeholder in Quality Improvement activities.
Collaborates with Learning and Development partners to ensure that educational content meets relevant internal standards and is available in a variety of channels based on the needs of the organization.
Considers the physical, psychological and socio-economic needs of the client and family and the ability of client needs to be met in the home.
Directs, plans and initiates action independently and responsively in home care situations.
Completes medical records to ensure compliance with standards of timeliness, accuracy, and completeness.
Communicates with local and enterprise resources and stakeholders as needed.
Communicates with clients primary care physician and other care providers as needed.
Qualifications:
Graduate of an accredited nursing school. BSN preferred.
Minimum of 3 years clinical nursing experience, preferably in home health/home care setting.
RN licensure and ability to obtain additional states of licensure.
Knowledge and Skills:
Experience working with medically fragile clients, addressing medical, social and behavioral components of care.
Ability to work independently and leverage community resources to benefit clients.
Excellent interpersonal, written and verbal communication skills.
Physical Requirements:
Typical office setting, indoor and extensive use of computer
Minimal lifting, bending but role typically requires long period of sitting
