Duration: 12-Month Contract (with potential extension)

Job Description:

We are seeking a compassionate and organized Social Worker to join our Clinical Care Team. This role supports primary care providers in addressing patients' social determinants of health (SDOH) and improving access to community resources. Key responsibilities include conducting social needs assessments, managing referrals, coordinating case management activities, supporting prescription access, and maintaining up-to-date resource lists. The Social Worker will also assist with discharge planning, patient follow-ups, and act as a liaison between patients and providers. Attendance at provider meetings, community health councils, and collaboration with social service agencies is expected.

Requirements:

Strong understanding of SDOH and community-based resources

Experience with case management and patient advocacy

Excellent communication and documentation skills

Ability to work collaboratively within a healthcare team
