We are seeking a passionate and dedicated Primary Care Nurse Practitioner who is ready to make a significant impact on the lives of their patients. This permanent position will give you the opportunity to provide comprehensive primary care services to a diverse patient population. You will be working in a dynamic and supportive environment where your expertise in managing chronic illnesses will be highly valued. Responsibilities:
As a Primary Care Nurse Practitioner, your primary responsibility will be to provide exceptional patient care. Your duties will include, but are not limited to:
1. Providing comprehensive primary care services, including acute and chronic illness management, preventive care, and health promotion.
2. Evaluating patients' health, diagnosing and treating illnesses, injuries and chronic conditions.
3. Managing patients with chronic illnesses, adjusting treatment plans as necessary, and coordinating care with other healthcare professionals.
4. Educating patients and their families on disease prevention and healthy lifestyle habits.
5. Developing and implementing patient management policies and procedures.
6. Participating in quality improvement, management, continuing education, and patient care programs.
7. Assisting with the training and supervision of nursing staff and students.
Qualifications:
The ideal candidate for this position is a highly skilled, compassionate, and dedicated professional. The following qualifications are required for this role:
1. Master’s degree in nursing from an accredited nursing program.
2. Current licensure as a Nurse Practitioner in the state of practice.
3. A minimum of 1 years of experience as a Nurse Practitioner, with a focus on primary care and chronic illness management.
4. Exceptional clinical skills with an emphasis on diagnosing and managing a comprehensive range of health conditions.
5. Excellent communication skills, with the ability to effectively communicate complex medical information to patients and their families.
6. Strong organizational skills and the ability to manage a high-volume workload.
7. Demonstrated commitment to patient-centered care and the ability to build strong relationships with patients.
8. Current CPR or BLS certification.
9. Ability to work independently, as well as part of a multidisciplinary team.
