The Nurse Liaison functions to maintain resident census based on facility goals, develop and implement a marketing program for specific facility, act as a liaison between the facility and community, keep abreast of any changes to the Census and coordinate efforts of the Admission Committee and the facility. This position must also coordinate the entire admission process and assist the family with administrative questions.
QUALIFICATIONS
• Current LPN License in good standing in applicable state
• Experience in the admission process and census development preferred
• Experience, training and/or education in marketing
• Ability to deal effectively with discharge planners, case managers, physicians, other referral contacts, families, responsible parties, residents, and staff
• Demonstrate good customer service skills and public relations skills
• Demonstrated ability to speak and present publicly
• Demonstrated ability to work independently
• Available to travel within a fifty (50) mile radius of specific facility
• Must be knowledgeable in the areas of general nursing, rehabilitative and restorative nursing, medical practices, procedures and laws, regulations and guidelines governing long-term care
• Must be at least 18 years of age
• Must speak, read, write, and understand English
• Must be capable of performing the ESSENTIAL JOB FUNCTIONS and PHYSICAL AND SENSORY
A. ADMINISTRATIVE FUNCTIONS
1. Receive all inquiry information coming into facility either by phone, fax or personal visit.
2. Process incoming information i.e. transfer received information onto approved NHS inquiry form.
3. Call hospital, family or whoever needed to obtain any extra information not sent with original set of information.
4. When all appropriate information is gathered and placed on approved form, take information to Admission Committee Meeting.
5. Keep abreast with any changes to the resident status and coverage section, and notify other departments of those changes and implement room changes and all associated paperwork and documentation of room changes as needed.
6. Ensure that Levels I and II, preadmission x-rays and other related preadmission examinations are completed prior the admission.
7. Financial information should be done by financial specialist to avoid any misinformation. Each department that does admission paperwork should continue to do their own paperwork to help prepare for the assessment and care plan process.
8. Maintain status of received information on an inquiry log.
9. Conduct Admission Committee Meetings as needed, but not more than once a day. Admission Committee Members should include, but not be limited to, Admission Coordinator, Administrator, Financial Specialist, Social Worker, Therapy Team Leader, and Director of Nursing.
10. Evaluate residents as necessary by visiting hospitals or other sources to gather information to clear up any concerns facility may have about a potential admission. Any staff member that could be appropriate to help alleviate any concerns should also visit.
11. Retain all information on approved inquiries for resident file when they are admitted.
12. Retain all information on denied inquiries for approximately one (1) year with an attached statement for reason of denial. This is done in case facility is audited.
13. Inform all managers and Unit Nurse of all admissions in a timely manner.
B. MARKETING FUNCTIONS
1. Develop and implement a marketing program for specific facility.
2. Contact community sources on a regular basis based on a specific written marketing plan and be held accountable for the results of those contacts and change the plan based on these results.
3. Make daily visits to referral sources within a 50 mile radius of facility, spending the majority of the day outside of the facility in sales/marketing calls to gain admissions to the facility.
4. Conduct market research and make marketing suggestions to the Admission committee as a routine part of the job. He/she will coordinate marketing efforts of the Admission Committee and the facility staff.
5. Evaluate resident/family satisfaction with admission process and facility
6. Act as facility liaison and representative at community / civic organization functions and keep abreast of changes in the market and direct members of the Admissions Committee regarding their participation in civic organizations and functions.
7. Provide customer service training to staff as needed.
