Job Details

We are seeking a Nurse Practitioner for our Bakersfield location to join our interdisciplinary team and deliver patient-centered care to our senior participants.

Responsibilities:

Conduct initial and ongoing assessments of participants, including medical history, physical exams, and diagnostic evaluations.

Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary team.

Manage chronic illnesses and adjust medications as necessary to optimize patient outcomes.

Provide preventative care, health education, and counseling to participants and their families.

Coordinate referrals and consultations with medical specialists.

Ensure continuity of care by monitoring participants’ progress and updating care plans as needed.

Document care provided, ensuring timely and accurate medical records.

Participate in team meetings to discuss care plans and strategies for patient well-being.

Serve as a resource for the interdisciplinary team regarding geriatric care.

Qualifications:

Certified Nurse Practitioner (NP) licensed to practice in California.

At least 1 year of experience working with an elderly or frail population preferred.

Strong skills in patient assessment, chronic disease management, and geriatric care.

CPR/BLS certification required.

Up-to-date immunizations and medical clearance for communicable diseases.
