Job Description Certified Nurse Assistant (CNA), + Home Health Assistant (HHA)

We are looking for a dedicated and compassionate CNA/HHA to join our team to provide direct patient care in a home setting. This role involves assisting patients with daily living activities, ensuring comfort and safety, and providing companionship while working under the supervision of a Registered Nurse (RN) or healthcare professional. The ideal candidate will support each clients unique goals and ensure each client is well cared for, and feel truly seen, and heard.
Duties and Responsibilities:
- Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and feeding
- Provide mobility support, including; physical therapies or exercises, ambulation, transfers, and positioning in bed or a wheelchair
- Assist with light housekeeping duties such as laundry, meal preparation, and maintaining a clean environment
- Instruct and counsel patients and families on diets and exercise
- Monitor (vital signs; temperature, respiration, etc) and report any changes in patients condition to the supervising nurse or healthcare professional
- Collect specimen
- Transport and accompany patients to doctors office appointment or hospitals
- Provide companionship and emotional/psychological support to patients to enhance their overall well-being
- Follow care plan established by the healthcare provider and maintain accurate records and documentation of all services provided
- Assist with medication reminders as per the care plan
- Adhere to all infection control protocols, safety guidelines, and healthcare regulation
Qualifications:
- High school diploma or equivalent required
- Certification:
o CNA: Certified Nurse Assistant Certification with valid state license in the state of VA
o HHA: Home Health Aide from a state-approved training program in the state of VA
- Prior experience in home healthcare, hospice, or assisted living is preferred but not required
- Compassionate and patient-centered approach to care
- Strong inter-personal and communication skills with proficiency in English required
- Ability to work independently and follow care plans effectively
- Ability to monitor vital signs and collect specimens
- Familiarity with basic nutrition and personal hygiene standards
- Physical ability to lift and assist patients as needed
- Caring and compassionate personality
- Ability to document all activities and support provided to patient
- Ability to use EMR software like AloraPlus is an advantage
- CPR certification is required
- First Aide training is required
- Current TB or Chest X-Ray
- Eligibility to work in the US
- Valid drivers license

Job Type: Part Time and Full Time roles Available
Pay Rate: $14.00 $22.00/hr. Rates are negotiable based on applicants experience
