Description of the role:

Regency Care of Silver Spring is seeking a dedicated Hospital Liaison to facilitate communication and coordination between hospitals and our facility. The Hospital Liaison will ensure seamless transitions for patients from the hospital to Regency Care, providing support and guidance throughout the process.

Responsibilities:

Establish and maintain relationships with hospital staff

Coordinate patient transfers and admissions

Ensure accurate and timely communication between hospital and Regency Care teams

Provide support and assistance to patients and families during the transition process

Collaborate with medical and administrative staff to optimize patient care

Requirements:

Strong communication and interpersonal skills

Previous experience in a healthcare setting preferred

Knowledge of hospital procedures and regulations

Ability to work independently and as part of a team

Attention to detail and organizational skills
