Cardiology Outpatient Advanced Practitioner

Trinity Health Of New England seeks a Nurse Practitioner or Physician Assistant to join the Cardiology team.

Join an established, growing cardiology interdisciplinary team of board-certified physicians and advanced practice providers

Daily responsibilities include:

See outpatients in office-based settings throughout the Hartford area, one outpatient center per day with Saint Francis Hospital as the hub.

Provide transitional care management

Offer guideline directed medical therapy optimization

Perform advanced evaluations, nuclear and stress testing.

Provide same day access care

The successful candidate:

Is interested in cardiac care

Has Cardiology experience (preferred but not required.)

Will have an Advanced Practice Nurse or Physician Assistant CT state license.
