What You’ll Do:
As an Client Intake Coordinator, you’ll be the first point of contact for prospective clients and families, guiding them through the care process with empathy and expertise. Your role is key in ensuring a seamless transition from initial inquiry to care services.
Key Responsibilities:
Respond to inquiries and schedule care consultations
Assess client needs and coordinate appropriate care plans
Manage intake paperwork, documentation, and onboarding
Build and maintain strong relationships with clients, caregivers, and community partners
Track referral pipelines and follow up on leads
Participate in a rotating on-call schedule to support client and caregiver needs

Who We’re Looking For:
High school diploma required; bachelor’s degree preferred
Strong computer skills (Microsoft Office & other software)
Excellent communication & organizational skills
