Job Description

JOB SUMMARY:

Serves as a clinical liaison and navigator for patients undergoing CT lung screenings or those identified with incidental radiologic findings, guiding them through the evaluation and follow-up process.

Reviews radiology results in collaboration with primary care providers and communicates findings and recommended next steps to patients in an empathetic, clear, and timely manner.

Educates patients about radiologic findings, diagnostic implications, recommended procedures, and follow-up plans to support understanding and engagement in care.

Coordinates and schedules follow-up diagnostic imaging procedures and other recommended services, minimizing delays in care while collaborating with the Network Navigation team.

Collaborates closely with an interprofessional team—including radiology, pulmonology, oncology, and primary care—to facilitate seamless transitions and reduce gaps in communication.

Actively participates in performance improvement efforts, process optimization, and patient satisfaction initiatives.

Maintains strong patient-provider relationships by demonstrating professionalism, compassion, and responsiveness throughout the patient journey.

The nurse navigator is responsible for developing competence in radiologic procedure nursing care. They collaborate closely with interdisciplinary teams to prevent delays in care and provide appropriate nursing coverage for procedures as needed.

Ability to work autonomously.

Certification/Licensure:

Current IN Registered Nurse (RN) license

Current BLS certification

Current ACLS certification

ADDITIONAL EDUCATION AND EXPERIENCE REQUIREMENTS:

Graduate of an accredited nursing program

Previous case manager or navigator experience preferred.

Program development experience strongly preferred.
