Health Center Outreach Specialist Team Lead

Full Time / Days / up to $25 per hour

Lutheran Social Services of Central Ohio is currently seeking a Health Center Outreach Specialist Team Lead for Faith Mission in Columbus, Ohio. Faith Mission is one of the largest homeless shelters in Columbus, Ohio, providing meals and shelter to men, women and Veterans with dignity and compassion.

What will I do as the Health Center Outreach Specialist Team Lead with Lutheran Social Services?

Develop and implement outreach strategies that increase community awareness and engagement

Coordinate and plan community events

Maintain partnerships with organizations including local public health and community-based organizations

Track and analyze outreach metrics and performance indicators

Serve as a liaison between the community and the health center

Promote access to primary care, behavioral health, dental and enabling services

Ensure accurate and timely documentation of outreach efforts and client interactions

Prepare data for reports for internal, grant compliance and board updates

Coordinate specialized and clinically orientated trainings to outreach team

Requirements for the Health Center Outreach Specialist Team Lead with Lutheran Social Services:

Minimum of two years in community outreach, public health or health education

At least one year of supervisory or team leadership experience

Experience working with the following or related populations: PLWHA, HOPWA, unstable housing, behavioral health, and or substance use disorder

Valid Ohio driver's license, access to reliable insured transportation and ability to meet LSS insurance standards
