Position Summary:

We are seeking a compassionate and experienced Director of Case Management to lead our case management department. This leadership role is responsible for overseeing the coordination of comprehensive, individualized care plans and discharge planning for patients with complex medical, cognitive, and behavioral needs. The Director will supervise a team of case managers and collaborate with interdisciplinary teams to ensure continuity of care from admission through discharge and community reintegration.

Responsibilities:

Lead, mentor, and supervise the case management team, ensuring compliance with regulatory standards and best practices.

Develop and monitor individualized patient care and discharge plans in collaboration with clinical and therapeutic teams.

Serve as the liaison between patients, families, insurance providers, and external partners.

Ensure accurate documentation, eligibility, authorizations, and timely communication with funding sources.

Facilitate family meetings, team conferences, and community resource referrals.

Track and report key performance indicators related to length of stay, discharge success, and satisfaction.

Assist with staff training, quality assurance initiatives, and continuous improvement projects.

Qualifications:

Bachelor's degree in Nursing, Social Work, or related field required (Master's preferred).

Current RN or LCSW license in the state of [State].

5+ years of experience in case management, with at least 2 years in a leadership role.

Experience working in neuro-rehabilitation or post-acute care settings strongly preferred.

Strong knowledge of discharge planning, managed care, and public assistance programs.

Excellent communication, organizational, and leadership skills.
