Job Summary: The Nurse Practitioner (NP) specializing in house calls provides high-quality, patient-centered care to individuals in their homes, particularly focusing on those with chronic illnesses, mobility issues, or limited access to healthcare facilities. This role emphasizes holistic care, patient education, and the management of complex medical conditions.
Responsibilities
Responsibilities include:
Patient Evaluations: Conduct comprehensive assessments of patients’ health status, including physical examinations, medical history reviews, and medication reconciliations during home visits.
Diagnosis and Treatment: Diagnose medical conditions, develop and implement individualized treatment plans, and prescribe medications as needed.
Care Coordination: Collaborate with physicians, specialists, and other healthcare providers to ensure continuity of care and comprehensive support for patients.
Patient Education: Provide education to patients and their families regarding health management, lifestyle modifications, disease prevention, and medication adherence.
Documentation: Maintain accurate and thorough documentation of all patient interactions, including assessments, treatments, and follow-up care in electronic health records.
Follow-Up Care: Schedule and conduct follow-up visits to monitor patient progress and adjust treatment plans as necessary.
Emergency Response: Assess and manage urgent medical issues during home visits, providing referrals or interventions as needed.
Community Engagement: Participate in outreach efforts to promote health services and educate the community on available resources.
Qualifications
Job Requirements:
Master's Degree in a Nurse Practitioner Program: Family or Adult Geriatrics
Current New York State License as a Nurse Practitioner.
National Board Certification require
American Association of Nurse Practitioners (AANP)
American Nurses Credentialing Center (ANCC)
Oncology Nursing Certification Corporation (ONCC)
Bilingual (Spanish Speaking) is a plus
EMR/EHR experience a plus
Work Environment:
Primarily operates in patients’ homes, which may vary in living conditions.
Requires travel within a designated geographic area, with a flexible schedule to accommodate patient needs, including possible evenings and weekends.
May involve working collaboratively with a multidisciplinary team.
Additional Information
A driver will meet the provider where the patients are located (a meeting point can be provided. No house pickups) 12 patients a day face to face in each borough assigned area, cluster patients in each borough. 200-300 patients for each provider. 8:30am starting until you reach the daily quota. number of working days by 10 then it will be 55/per patient.
