Licensed Master Social Worker – Hospital Setting (Contract $66/hour Weekdays Only)

Location: Manhattan & The Bronx, NY

Schedule: Monday–Friday, 9:00am–5:00pm

Pay Rate: $66/hour

Licensed Master Social Worker (LMSW) Responsibilities:

Review admissions to assess patient needs, including religious or dietary accommodations

Act as a liaison between patients and the care team to advocate for individual needs

Provide discharge counseling to ensure safe transitions from hospital to home

Refer patients to services addressing homelessness, food insecurity, unemployment, and transportation

Coordinate care between medical providers for complex and high-need cases

Licensed Master Social Worker (LMSW) Qualifications:

Master’s Degree in Social Work (MSW)

Licensed Master Social Worker (LMSW) in New York State

Minimum 1–2 years of hospital social work experience

Knowledge of NYC social services and community-based resources

Strong clinical judgment and excellent communication skills
