Join our dynamic team at Valley VNA Senior Care as our Memory Care Coordinator within our assisted

living community. This newly-created position offers the unique opportunity to enhance our culture of

engagement and support for those living with dementia in our assisted living community. Working

collaboratively with our residents, their families, our team members, and volunteers, you will coordinate

and facilitate our memory care and culture change initiatives. You will have the opportunities to create

and facilitate innovative memory care life enrichment programs, to collaborate with our team member

to learn these skills, and to cultivate our environment of person-directed life.

As a Certified Alzheimer's Disease and Dementia Care Trainer, you'll lead a dedicated team, fostering a supportive environment where empathy and integrity thrive. As our full-time Memory Care Coordinator at Valley VNA Senior Care Assisted Living, you will utilize your

therapeutic techniques to enrich the lives of our residents, their families, and our team members. Your

role includes therapeutic recreation/life enrichment program facilitation that directly benefits our

culture of engagement and support. Collaborating closely with our resident care team, you will provide

learning opportunities that complement our memory care and culture change initiatives. You will also

be a shared leader in these opportunities with other dedicated tea members.

To excel as our full-time Memory Care Coordinator, you will need a robust set of skills that align with our

core values of Trust, Communication, Safety, and Teamwork. A Bachelor's Degree in Therapeutic

Recreation or a related field is preferred along with extensive experience in working with persons living

with dementia.

Strong communication and interpersonal skills are essential, empowering you to build and maintain

rapport with those we serve. Collaborative and shared leadership skills are necessary to as you support

in the learning of our memory care best practices and culture initiatives.

Additionally, experience in leading and motivating team members is vital for inspiring a collective embrace of memory care and culture change standards of excellence. Successful candidates will demonstrate a passion for nurturing a supportive environment that prioritizes our residents' well-being while actively contributing to a collaborative and dynamic team culture.

Key Responsibilities

Develop and implement memory care-specific life enrichment programs

Lead, support, and educate on the best practice approaches to engagement for persons living

with dementia

Embrace and share leadership in our culture transformation initiatives

Work collaborative with the resident care and clinical teams for personalized plans of care

development and implementation

Embrace an active participant in care conference and family communication and support

Established and maintains relationships with family members by providing memory care-specific

education, support, and resources

Collaborates with the Life Enrichment team to enhance memory-care specific best practice

engagements

Ensure adherence to all relevant regulations, policies, and procedures

Skills and Qualifications

Bachelor's degree preferred in therapeutic recreation or related fields

Extensive experience in memory care and best practice implementations

Demonstrates understanding of the progressive disease of Alzheimer's and all other related

dementias.

Excellent verbal and written communication skills

Experience with leading and motivating team members on embracing memory care and culture

change standards of excellence
