RN Med/Surg PRN- Appleton

Currently offering PRN/Part Time PTA positions in Port Clinton, OH to serve homebound patients.

Candidates must hold a valid Ohio PTA license and be able to travel to patients' homes.

The role involves daily communication with office staff for patient intake, paperwork, and payroll.

Maintain open communication with patients and home health agencies to ensure quality care.

Collaborate with Lead Physical Therapist to manage patient treatment plans.

Provide in-home therapy, educate patients and caregivers, and complete detailed documentation.

Attend team meetings as needed.
