The Home Health RN Case Manager provides personalized in-home nursing care to enhance patient quality of life.

Responsibilities include conducting in-home evaluations, directing skilled nursing services, and supporting the patient's overall Plan of Care.

Focuses on patient education and resource provision to enable self-management and safe at-home living.

Requires a graduate of an accredited nursing program, an active RN license in Missouri, and a valid driver’s license.

Preferred candidates have at least one year of recent nursing experience.

Must obtain BLS certification within 90 days and have reliable transportation.
