CDL A Truck Driver

Seeking a High Acuity Social Worker to support individuals living with HIV who face social barriers affecting care engagement and viral suppression.

Responsibilities include conducting assessments, crisis intervention, developing and overseeing service plans, and providing 1:1 harm reduction therapy.

Supervise High Acuity CHWs, guide case management, and ensure timely documentation.

Collaborate with care teams, participate in supervision and training, and adhere to confidentiality and agency policies.

Requirements: MSW + LCSW in MA, 2+ years experience, cultural competence, and commitment to equity.

Preferred: supervisory experience, LICSW, trauma-informed approaches.
