The Registered Nurse (RN) is part of an interdisciplinary team providing comprehensive hospice care to patients and families facing end-of-life and bereavement.
Responsibilities include care coordination, assessments, education, supervision of delegated tasks, and participation in team meetings and quality improvement activities.
Performs comprehensive physical, psychosocial, spiritual, and bereavement assessments to manage symptoms and develop individualized care plans.
Ensures documentation, complies with policies, and maintains infection control practices. Participates in on-call rotations and provides patient-centered nursing services in various settings.
Supports professional growth through training, orientation, and active participation in quality initiatives. Meets licensure requirements, has reliable transportation, and at least two years of relevant nursing experience.
Physical demands include lifting, walking, and visual acuity, with accommodations available. The RN reports to the Team Manager and works to deliver compassionate, quality hospice care.
