CDL-A Independent Contractor Truck Driver

Seeking a full-time Advanced Practice Provider (Physician Assistant or Nurse Practitioner) to deliver comprehensive medical care to long-term and short-term rehab residents.

Salary ranges from $114,000 to $125,000 based on experience.

Provides high-quality, person-centered care in collaboration with physicians and the interdisciplinary team.

Supports residents and families with communication, education, and end-of-life care.

Ensures documentation, billing, and compliance with regulations.

Participates in on-call rotations, committees, and quality improvement initiatives.

Requirements include a Master’s degree or higher, NYS licensure, and 2+ years of geriatrics or long-term care experience preferred.
