The RN Care Manager role serves Licking and Knox counties, offering a full-time schedule (8:00A-4:30P) with weekend and holiday rotations.

Responsibilities include planning and delivering patient care through assessment, planning, interventions, implementation, and evaluation, while collaborating with colleagues and maintaining professional standards.

Qualifications include RN licensure in Ohio, graduate of an accredited nursing school, CPR certification, computer skills, and a valid driver’s license with a good driving record. Preferred candidates have two years of acute care experience and Hospice/Palliative Medicine certification.
