Respiratory Therapist

Join a compassionate, nurse-led team dedicated to delivering exceptional end-of-life care with purpose and support.

Provide social work services to hospice patients and families, including assessments, counseling, and care planning.

Complete evaluations within 5 days of admission and re-evaluate needs regularly.

Assist with legal, financial, funeral, and advanced directives planning.

Educate families about community resources and cultural considerations.

Offer bereavement support and respond to psychosocial needs.

Qualifications include a Master’s in Social Work, valid state license, healthcare experience, and a valid driver’s license. Preferred experience with hospice and familiarity with HCHB are a plus.
