Licensed Vocational/Practical Nurse (LVN/LPN)

Seeking a PRN Nurse Practitioner for home risk assessments, including adult/geriatric evaluations, social and medical history reviews, and care gap identification.

New grads welcome; flexible schedule with days, evenings, nights, and weekends.

Conduct assessments in various environments like homes and skilled facilities, collaborating with healthcare teams.

Compensation ranges from $1,900 to $9,500/month based on visit volume.

Requirements include a Master’s or equivalent experience, current RN and NP licensure with prescriptive authority, board certification, and 1+ year NP experience.

Travel required to patient locations with valid driver’s license and insurance.
