Nursing Assistant (NA) In-Training

Seeking a full-time Nurse Practitioner (NP) for home risk assessments, welcoming new grads.

Conduct comprehensive assessments in patients' homes, skilled facilities, or community settings in Salem, VA.

Evaluate health, medical history, medication adherence, and social factors to improve care and close health gaps.

Collaborate with physicians, social workers, and caregivers, providing follow-up and health education.

Position offers a $7,500 sign-on bonus, competitive salary ($102,300-$125,900), flexible schedule, and benefits including medical, dental, vision, 401K, PTO, and mileage reimbursement.
