This full-time MDS Coordinator (RN/LPN) position involves conducting and coordinating resident assessments in compliance with state regulations and facility policies.
Responsibilities include assessing residents regularly, communicating condition changes, ensuring completion of assessments and care plans, and updating documentation accordingly.
Coordinate with interdisciplinary teams to develop and distribute MDS schedules, transmit assessments to the state, and maintain accurate records.
Qualifications include a valid RN or LPN license and at least 1 year of experience in Long-Term Care as an MDS Assessment Coordinator.
