Description:
The Licensed Practical Nurse (LPN) provides direct and indirect home care to clients, following organizational policies and state regulations.
They educate, motivate, and assist clients in health maintenance and disease prevention to promote independence at home.
Responsibilities include administering medications, documenting care and patient progress, performing assessments, and communicating with families and physicians.
They conduct supervisory visits, ensure regulatory compliance, and support care planning and staff training.
The role requires a nursing degree, valid licensure, CPR certification, and preferably home health experience.
Strong communication, organizational skills, and the ability to work independently or with a team are essential.
The position offers $29/hour and involves flexible hours, including on-call and holiday shifts, to meet client needs.
