Physical Therapist bonus

We are seeking a Nurse Practitioner (PRN) for home risk assessments, welcoming new grads.

Conducts comprehensive adult/geriatric assessments in home, SNF, and community settings, reviewing medical history, social factors, and risks.

Collaborates with physicians, case managers, and caregivers to improve patient care and close health gaps.

Provides follow-up, health education, and referrals, documenting according to protocols.

Flexible schedule with day, evening, night, and weekend hours, earning $1,900–$9,500/month based on assessments completed.

Requires RN and NP licensure, certification, prescriptive authority, and at least 1-year NP experience.

Travel involved to patient homes with a valid driver license and insurance.
