Physical Therapist

We are seeking a Nurse Practitioner (PRN) for home risk assessments, welcoming new graduates.

Conduct comprehensive assessments of adult and geriatric patients in home, SNF, and community settings, reviewing medical history, social factors, and risks.

Collaborate with physicians, social workers, and caregivers to develop care plans, provide health education, and facilitate follow-up.

Assessments last up to one hour, aiming to improve care quality and address care gaps.

Flexible scheduling including days, evenings, nights, and weekends, with travel to patient homes in Palestine, TX.

Earn $2,000-$10,000/month based on completed assessments, with mileage reimbursement.

Requirements include a Master’s degree or equivalent, current RN and NP licensure, board certification, BLS/ACLS, and computer skills.

Travel is necessary; valid driver’s license and auto insurance required.
