The PHP Clinician conducts patient evaluations, assesses presenting issues, formulates diagnoses, and recommends treatment plans.
Assists with developing, implementing, and modifying treatment and continuing care plans, while providing case management and discharge summaries.
Provides crisis management, interacts with referral sources, and participates in ongoing education and supervision.
Ensures documentation complies with standards, monitors caseload for payer requirements, and serves as a community resource through presentations and consultations.
Mentors interns and students, adheres to ethical guidelines, and performs additional duties as assigned.
Requires a Master’s degree, 1-2 years supervised counseling experience, familiarity with relevant technology, and an independent license (preferred).
