Nursing Supervisor, RN (Irondequoit Campus)

The COPD Navigator role supports patients from diagnosis through recovery by providing education, care coordination, and referral recommendations.

Under supervision, ensures timely inpatient and ER consultations, recommends pulmonary rehab and PFTs, and provides comprehensive inhaler education.

Collaborates with a multidisciplinary team, reviews medical records via EPIC, and conducts follow-up calls post-discharge.

Utilizes educational tools like handouts, lung models, and videos; collects data on patient interactions and referrals; and participates in community outreach.

Qualifications include an associate degree in respiratory care (bachelor's preferred), 4+ years of clinical experience, and certifications such as RRT, BLS, and specialized COPD training.

Requires the ability to operate vehicles for work purposes as needed.
