Home Health Physical Therapist Assistant - Part Time

This full-time Practical Nurse position based at Rockbridge Community Hospital in Lexington, VA offers evening/night shifts from 7pm-7am with every other weekend work.

Provides practical nursing under RN or physician supervision, assisting with patient data collection, planning, and evaluation across all ages.

Contributes to developing care plans, identifies educational needs, and implements prescribed treatments and medications.

Acts as a patient advocate, safeguarding rights and offering psychosocial and cultural support.

Participates in performance improvement and supports team development.

Requires graduation from an accredited practical nursing program, VA licensure, BLS certification, and de-escalation training within four months.

Experience preferred; must complete orientation and meet essential job functions.
