Licensed Mental Health Therapist - Cumming

We are seeking a Nurse Practitioner (PRN) for home risk assessments, open to new grads.

Conducts comprehensive assessments during up to one-hour visits, reviewing health, medical history, medication adherence, social factors, and risks.

Collaborates with physicians, case managers, and caregivers to improve patient care and close care gaps.

Works in various settings, including home and skilled facilities, providing health education, diagnosis, treatment, and documentation.

Flexible schedule with options for days, evenings, nights, and weekends.

Requires a Master’s degree or equivalent experience, current NP licensure, board certification, and at least 1 year of NP experience.

Must have valid driver’s license, reliable transportation, and willingness to travel to patient homes in Olean, NY.
