Pediatrics RN/LPN - Rome

The Home Health Occupational Therapist evaluates, plans, and administers personalized occupational therapy in patients' homes to restore and improve daily functioning based on physician prescriptions.

Provides initial and ongoing assessments to determine patient capabilities and develops in-home treatment plans in collaboration with the care team.

Implements evidence-based interventions, educates patients and families, and makes referrals as needed.

Supervises OT Assistants and completes necessary documentation per regulations.

Qualifications include a valid Illinois OT license, NBCOT certification preferred, CPR certification, and reliable transportation. Experience in home care or hospital settings is preferred.

Strong communication, organizational skills, and professionalism are essential. The role involves working in diverse environments and maintaining HIPAA privacy standards.
