Occupational Therapist - Casual/PRN

The Physical Therapist (PT - HH) role focuses on restoring function and minimizing disability following illness or injury, helping clients achieve maximum performance within their capabilities.

Responsibilities include evaluating and treating patients with medically prescribed programs, supervising Physical Therapy Assistants, conducting initial assessments, and documenting observations, interventions, goals, and discharge plans.

The therapist will notify the Clinical Team Manager of OASIS scores and perform other duties aligned with skills and mission.

Territory includes Milpitas, Saratoga, Los Gatos, San Jose, Morgan Hill, Sunnyvale, and neighboring areas.

Requirements include at least one year of Home Health experience.
