Speech Language Pathologist (Soaring Heights Schools)

This RN position involves planning and delivering hospice care in patients' homes, emphasizing community health education.

Work Monday to Friday, 8:00 AM - 5:00 PM, with on-call as needed.

Provides comprehensive assessments, develops and revises care plans, administers medications, and coordinates specialized medical equipment.

Communicates effectively with patients, families, physicians, and interdisciplinary teams to ensure quality care and education.

Acts as Case Manager when assigned, ensuring continuity of care and emotional support.

Requires at least 2 years of pediatric hospice RN experience, recent RN experience in acute and home care, and certification in hospice and palliative care (CHPN).

Must hold a current nursing license, CPR and PALS/PEARS certifications, a valid driver’s license, and have reliable transportation.

Preferred candidates have a bachelor’s degree and hospice care experience.

Position involves prolonged walking, standing, lifting, and bending; must have good visual and hearing acuity.

Multiple locations on Oahu, HI; full-time, pay $40-$50/hr, with benefits including health, dental, vision, 401(k), PTO, and insurance.
